Before opening the document given in the link at the bottom of the page, some security adjustments on macros are necessary in Microsoft Word.

 

STEP 1: Open Tools menu, then click on Options...

 

 

STEP 2: In Options, activate the Security tab. In Security tab, click on the Macro Security... button.

 

 

STEP 3: Assure that, Security Level tab is activated. Activate either the Medium or Low security option. 

 

 

STEP 3 (continued): Activating Low security level and readjusting the security level after filling out the form would be easier for the user.

 

STEP 4: If Medium security level is chosen, please click on the Enable Macros button when opening the templates given in the link at the bottom of the page.
